Therapeutic Effect

Neurazine (chlorpromazine hydrochloride – Egypt) is a potent medication that can create a state of calm and reduced motor activity at doses that do not induce sleep. It inhibits the vomiting center and regulates temperature. It also enhances the effectiveness of analgesic [acting to relieve pain] medications.

Indications

  • Sedative for nervous tension [A feeling of worry, nervousness, or unease, typically about an imminent event or something with an uncertain outcome] associated with some nervous and psychological diseases.
  • Antiemetic [A drug that is effective against vomiting and nausea] for uremia [A condition involving abnormally high levels of waste products in the blood], enteritis [Inflammation of the small intestine], diabetes [A chronic metabolic disorder characterized by elevated blood glucose levels], nausea, and vomiting resulting from taking certain medications to treat hiccups, heatstroke, and severe itching.

Contraindications

  • Hypersensitivity to phenothiazines [A class of typical antipsychotic drugs].
  • Do not use in cases of fainting or in the presence of large amounts of central nervous system depressants [Substances that can slow brain activity] (alcohol, barbiturates [A type of central nervous system depressant], and narcotics [A class of drugs that include opioids]) and treatment of glaucoma [A group of eye diseases that damage the optic nerve].

Side Effects

It is possible to experience extrapyramidal symptoms [Drug-induced movement disorders] secondary to the use of thioridazine [A typical antipsychotic drug], which are:

  • Parkinsonian tremor [A tremor resembling that seen in Parkinson’s disease].
  • Dystonia [A movement disorder in which muscles contract involuntarily, causing repetitive or twisting movements].
  • Akathisia [A state of agitation, distress, and restlessness].
  • Dyskinesia [Abnormality or impairment of voluntary movement].

Adverse effects depend on the dose and include decreased blood pressure and disturbances in body temperature regulation, which may lead to serious side effects in the elderly.

Other Side Effects:

  • Drowsiness, apathy [Lack of interest, enthusiasm, or concern], tension, agitation, insomnia [Persistent problems falling and staying asleep], convulsions [Sudden, uncontrolled electrical disturbance in the brain], dizziness, headache, gastrointestinal disorders, cardiovascular disorders [Diseases involving the heart or blood vessels] (such as high blood pressure, increased heart rate, and irregular heartbeat).
  • Hormonal disorders.
  • Blood cell disorders.

Note:

Intramuscular [Situated in or performed by injection into a muscle] injection of this drug may be painful and may cause an increase in blood pressure and increased heart rate.

Warnings

It is best to avoid most psychiatric medications during pregnancy unless absolutely necessary, and breastfeeding is not recommended during treatment.

The patient is advised not to drive or use machinery during the first days of treatment with Neurazine.

Use with caution in the following cases: the elderly, patients suffering from weakness, patients with a history of liver and bile duct failure, the presence of a deficiency in white blood cells, arteriosclerosis [The buildup of fats, cholesterol and other substances in and on the artery walls], cardiovascular diseases [Diseases involving the heart or blood vessels], or in other cases in which a sudden drop in blood pressure is undesirable.

Do not use antipsychotics [A class of medication primarily used to manage psychosis] to treat dementia-related psychosis [A set of behavioral and psychological symptoms related to dementia] , as elderly patients with dementia-related psychosis who are treated with antipsychotics are at higher risk of death.

Use During Pregnancy

Non-teratogenic effects [Effects that do not cause birth defects]:

Newborns exposed to antipsychotics during the third trimester of pregnancy may experience extrapyramidal [Drug-induced movement disorders] and withdrawal symptoms after delivery. Some cases have been reported in which these infants experienced agitation, hypertonia [Increased muscle tone], hypotonia [Decreased muscle tone], tremors, drowsiness, respiratory distress, and feeding disorders. These complications varied in severity, and in some cases the symptoms were self-limited, while in other cases newborns required admission to the intensive care unit and prolonged hospitalization.

Chlorpromazine hydrochloride should only be used during pregnancy if the potential benefits justify the potential risks to the fetus.
