Pharmacological action
DOLO-D® PLUS is well balanced formula consists of:
1- Ibuprofen
Ibuprofen is Analgesic, antipyretic, anti inflammatory, it is an NSAIDs which is believed to work through inhibition of cyclooxygenase enzyme (COX) thus inhibiting prostaglandin synthesis.
- Approximately 80% of an oral dose of ibuprofen is absorbed from the GI tract, Absorption rate is slower and plasma concentrations are reduced.
- Ibuprofen is metabolized via oxidation.
- Excretion of ibuprofen is essentially complete within 24 hours following oral administration. About 50-60% of an oral dose is excreted in urine as metabolites within 24 hours. Less than 10% of the drug is excreted in urine unchanged; the remainder of the drug is eliminated in feces.
2- Pseudoephedrine hydrochloride
Potant vasoconstrictor equal to ephedrine with mild side effects. Pseudoephedrine is a sympathomimetic amine-that, its principal mechanism of action relies on its indirect action on the adrenergic receptor system. When the adrenergic receptor activated by pseudoephedrine, the muscles contract, causing the blood vessels to constrict (vasoconstriction). The vasoconstriction that pseudoephedrine produces is believed to be principally an adrenergic receptor response. Pseudoephedrine to some extent, has decongestant action, pseudoephedrine shows greater selectivity for the nasal mucosa and a lower affinity for central nervous system (CNS) adrenergic receptors than other sympathomimetic amines.
- Pseudoephedrine hydrochloride is completely absorbed from the GI tract.
- pseudoephedrine is presumed to cross the placenta and to enter CSF.
- Pseudoephedrine distributes into breast milk; about 0.5% of an oral dose isdistributed into breast milk over 24 hours.
- Pseudoephedrine is incompletely metabolized (less than 1%) in the liver by N demethylation to an inactive metabolite ,
- The drug and its metabolite are excreted in urine; 55-96% of a dose is excreted unchanged.
3- Chlorpheniramine maleate
Chlorpheniramine maleate is Antihistamine agent of short duration of action with mild sedative effect
- Chlorpheniramine maleate: Appears to be well absorbed following oral administration; however, the drug undergoes substantial metabolism in the GI mucosa during absorption and on first pass through the liver.
- Chlorpheniramine and its metabolites are apparently excreted almost completely in urine.
Precautions & Warning
Cardiovascular Risk: NSAIDs may cause an increased risk of serious cardiovascular thrombotic events, myocardial infarction, and stroke, which can be fatal. This risk may increase with duration of use. Patients with cardiovascular disease or risk factors for cardiovascular disease may be at greater risk. NSAIDS are contraindicated for the treatment of peri-operative pain in the setting of coronary artery bypass graft (CABG) surgery.
